In-office Dental Incentive Program
We are offering an in-office dental wellness program designed for patients without dental insurance. This is NOT a dental insurance plan. However, it will assist you in reducing your dental costs and help maintain your oral health. This plan is for established patients. Therefore, the new patients visit (exam, x-rays, cleaning) is not covered under this plan.
Annual Fee
$369 – Adult
$199 – Children Under 13
$445 – Adult Perio
The Annual Fee includes the following Preventative Services:
- 2 Prophylaxis (Cleanings) or Periodontal Maintenance Visits
- 2 Oral Exams
- 1 Set of Bitewing X-rays
- 2 Fluoride Varnish Treatments (Under Age 18)
A 15% discount will be given on any additional treatment that is paid in full at time of service with cash, check. Discount does not apply when using Card Credit.
Provisions of the Program
- Initial fee is non-refundable
- This cannot be used in conjunction with an insurance plan
- Payment for additional treatment due at the time of service
- There is a $60 re-enrollment fee if the program lapses (30 day grace period allowed)
- There are no waiting periods and no deductibles
- Is non-transferrable to another party
- Benefits do not carry over to the next policy year
- Policy year begins when the fee is paid
